The U.S. Navy has taken responsibility for failures in its Navy SEAL training course that led to the death of New Jersey native Kyle Mullen last year. Details of those failures across multiple systems were released in a report this week that showed instructors were often ill-trained for their roles and left candidates "at high risk of serious injury."
